Wrist Fracture Treatment By Osteopaths

When the weather begins to get icy it gets less safe underfoot and people start to fall over and hurt themselves. A common injury is a fall on the outstretched hand (FOOSH) which often results in wrist fracture. When we say wrist fracture we are usually describing a fracture of the end of the radius and ulna, the two major bones of the forearm. Wrist fractures vary from very minor like a chip to major breaks which require operative fixation. Osteopaths work in fracture clinics and rehabilitate the hand, wrist and forearm after such injuries.

In the arm the wrist is the most commonly injured area and radius and ulna fractures make up 75% of all wrist fractures. Injuries vary from a single fracture which remains in place without displacement to severe injuries with many fragments (comminuted) and with the bones out of position. The age of the person dictates to some degree the type of fracture experienced: adults suffer radius and ulna fractures in the last inch of the forearm, adolescents displace the wrist growth plate and children suffer from a bend in the cortex of the bone called a greenstick fracture.

The commonest age groups for colles fractures to occur in are the 6-10 and the 60-69 year olds, with older people more likely to suffer fractures in the forearm away from the joint and younger people, due to the higher violence of the injury, being more likely to get joint involvement in the fracture. Diagnostic features of a radius and ulna fracture are significant pain with increased pain on palpating the area, a dinner fork bony deformity, swelling over the area and a marked reluctance to use it.

Orthopaedic Management of Wrist Fracture

To allow the fracture to heal correctly a colles fracture needs to be fixed in a position that allows the fracture to be held in as close to the original shape as possible. A simple fracture which is undisplaced can just be plastered and left to heal, while a displaced fracture has to be returned to a better anatomical alignment. Manipulation and plastering might work, but if the fracture does not remain in a good position then operative fixation with k-wires or plates and screws might be required. After the operation plaster is applied to maintain the correction.

Osteopathy after Wrist Fracture

The typical time in plaster is five to six weeks and once it comes off the osteopath can assess and rehabilitate the wrist and hand. The condition of the wrist and hand is very variable on coming out of plaster and a skilled assessment of the problems and potential for improvement is vital. If the osteopath determines that the wrist is uncomplicated after removal of plaster then they will prescribe mobilizing exercises for the wrist, forearm and hand and perhaps the elbow and shoulder. Coming straight out of plaster is a shock for the wrist and a strap on futura splint can rest the wrist and permit normal activity without too much discomfort. If the wrist is very stiff then attendance at a hand class may be useful and the accessory joint movements can be restored by using joint mobilization techniques on the many wrist joints. The physio will progress to strengthening the wrist as the movements improve and teach the patient to use the hand normally in daily activities.

One of the biggest hurdles for the studying of massage therapy is simply economic. Clinical studies are funded by companies hoping to profit from the positive results. There is not a way for a company to control massage techniques. This then leaves a void for funding these types of studies. However, these techniques have been used and refined since the beginning of time. They just don’t have the same documentation as a large pharmaceutical company.

The Neck Pain and Lower Back Pain Secret

The Complex Causes of Neck Pain and Lower Back Pain. The 2 most common forms of pain in the human anatomy are Neck Pain and Lower Back Pain, although the cause of either can be complex at times due to so many different causes. There is however, a root cause or secondary cause of Neck Pain or Lower Back Pain that is frequently overlooked, very difficult to diagnose, and oftentimes labeled as a secret or hidden cause. Neck Pain or Lower Back Pains hidden cause or secret source is known as Facet Joint Pain or Facet Syndrome. Facet Syndrome, or simply Facet Joint Pain is directly related to pain in the facet joints, and is a chronic pain syndrome, which is a collection of Chronic Neck Pain symptoms or Chronic Lower Back Pain Symptoms.

What does Articulation have to do with a Facet Joint? The posterior spinal joint that interfaces an adjacent upper and lower vertebrae together, is called a Facet Joint or Zygapophyseal Joint. Articulation within defined ranges of motion, is the medical term for Facet Joint movement. The articulation of a Facet Joint includes flexion, which is bending forward, extension, which is bending backward, and Cervical Rotation, or Thoracolumbar Rotation, which is twisting from side to side. Facet Joint unload and stretch when flexed, and load and compress when extended, while having complex combination of either when twisted to one side or the other. Luckily for us, Facet Joints also limit excessive range of motion, which can cause damage to spinal discs in either the Neck or Lower Back.

Are Facet Joints Synovial Joints? Synovial Joints are the most common joint in the human body, and Facet Joints are most definitely Synovial Joints. Synovial Joints are characterized by having a joint capsule with synovial fluid that lubricates the inside of the joint and by having cartilage on the outside of the joint between the lower adjacent surface and the upper adjacent surface of each vertebra to provide for smooth gliding movements.

What is Facet Arthritis? Facet Arthritis or Facet Arthropathy is a degenerative arthritis-facet joint inflammation that can be one of the syndromes of Facet Joint Pain. Both chronic pain conditions can be caused by wear and tear, degeneration of the cartilage surfaces, joint capsule or associated vertebral discs, injury or trauma, incorrect posture-excessive forward head posture, or abnormal spinal curvature such as Hyperlordosis-excessive arch in the lower back.

What could affect Joint Movement? Facet Joint movement or articulation should be smooth without restriction. If the joint becomes stuck or fixated in the open or closed position or in the partially open or partially closed position, then Facet Joint Pain is often the result. When muscles surrounding the Facet Joint Dysfunction become hypertonic-extremely tight and splint or support the problem, then additional pain is often experienced. The Facet Joint Dysfunction in itself can cause pain and the musculature that surrounds the joint could have a problem that is causing the joint to become stuck open or closed, resulting in Facet Joint Pain.

What else can cause Facet Joint Pain? Some causes of Facet joint pain are: Abnormal spinal curvature, Hyperlordosis-excessive arch in the lower back, Scoliosis-Lateral Lumbar spinal curvature, Injury or Trauma, Incorrect Neck Posture or incorrect Lower Back Posture or Forward Pelvic Rotation.

What are Facet Syndrome Symptoms of the Lower Back? Lower back pain or tenderness, increased pain upon side bending or twisting or arching the lower back or torso, deep dull aching pain that moves to the buttocks or the back of the thighs, and/or stiffness or difficulty with certain movements, such as standing up straight or getting up out of a chair are all symptoms of Lower back facet joint pain symptoms. The initial onset of pain might be related to a rotational spinal injury or a spinal injury involving hyperextension of the spine.

What are Facet Syndrome Symptoms for the Neck-Cervical? Neck symptoms for Facet Join pain include Headaches, Shoulder Pain, and/or difficulty side bending or rotating the head, as well as Neck Pain. The pain scale for the associated problems would be indicated by a Facet Joint Pain Assessment. A history of some kind of Neck Injury such as a whiplash could be the source of the problem.

An experienced Medical Massage Therapist will be able to formulate an effective exercise treatment plan.
